TEAMS FOR THE CURE FORM A TEAM OR JOIN A TEAM Teams can be formed ONLINE or OFFLINE (MAIL-IN TEAM ENTRY FORM).
All team members do not have to participate in the same event.
We request a minimum of TEN participants to form a team.(Paper team requirement only.)

To join an ONLINE Team:
- Select "Join a Team"
- Select the Team you wish to join
- If you can't find your team, contact your Team Captain
ALL team members will have their shirt delivery to the address provided at the time of their registration. Deadline for join team and having shirt mailed is May 4, 2008 OFFLINE TEAMS Deadline has passed.
Teams may register OFFLINE (paper entry form) by completing the TEAM REGISTRATION FORM and returning it to the Race office.
To form an OFFLINE Team:
- Download a Team Captain Packet and Roster form
- Download Team Captain Packet and Roster Short Form
- Download mail in entry form Needed for each team member
- Each Team member must complete and sign his/her own individual entry form
- Team Captain must collect all entry fees and registration forms from TEAM participants
- Team Captain must complete a Team Registration/Team Roster
- Submit ALL Team members completed entry forms, fees, donations and completed a Team Registration/Team Roster form in ONE envelope to:
